• 19 Dec 2002 | SN 2002(31) - Inappropriate use of breathing filters, HMEs and HMEFs in breathing circuits: risk of inadequate ventilation
  

  There is a risk of ineffective ventilation to the patient if breathing filters, heat and moisture exchangers (HMEs) or heat and moisture exchanging filters (HMEFs) incorporated into breathing circuits become occluded due to inappropriate use.

• 18 Dec 2002 | SN 2002(30) - Ureteric stent degradation by light
  

  The Cook Sof-Flex® Multi- length ureteric stent may degrade and become unfit for use before the indicated expiry date, if it has not been stored in the dark.

• 02 Sep 2002 | SN 2002(24) - Steam penetration tests in vacuum benchtop sterilizers
  

  A daily steam penetration test necessary to assist in the demonstration of the effectiveness of vacuum benchtop sterilizers that process wrapped, textile, hollow or tubular loads. Manufacturers of varios strilizers on the market do not provide information on the type of steam penetration test that is suitable and validated for their particular strerilizer. This could lead to failure to sterilize loads.

• 13 Mar 2002 | SN 2002(07) - Little Sister 3 and SES2000 Benchtop steam sterilizers (long chamber versions)
  

  Internal wiring on long chamber versions of Little Sister 3 and SES 2000 benchtop steam sterilizers (either with or without vacuum) can overheat. Acrid fumes or smoke might be emitted should the insulation melt or catch fire.
