They are equivalent to the MDA 'immediate action' category. They are reviewed every year and the list below contains notices that are still in force; if a notice has been withdrawn, it will no longer appear on these lists.

• 23 Oct 2002| HN 2002(07) - All respiratory therapy devices connected to endotracheal and tracheostomy tubes: risk of serious injury due to incorrect assembly

  There are an increasing number of reports of barotrauma to patients with endotracheal and tracheostomy tubes. Incorrectly assembled respiratory therapy devices, where an expiratory pathway was not provided, can result in the build up of excessive pressures causing lung injury.

• 23 Nov 2001| HN 2001(05) - Anaesthetic Breathing System Components: Risk of Blockages from Intravenous Administration (Giving) Set Caps

  The Medical Devices Agency (MDA) has received reports of intravenous administration set caps becoming lodged in breathing system components. Blockage can lead to death or serious permanent injury.
  Following frther investigation this information reierates and supplements the advice given in HN2001(02) published in August 2001 and HN2001 (03) published in September 2001.
