Problem:
Failure to defibrillate. The manufacturer is recalling these defibrillators to correct a malfunction that prevents or delays the delivery of therapy, which can lead to failure to resuscitate a patient.

Action by:
All medical and nursing staff, paramedics, resuscitation training officers, community defibrillator officers and technical staff responsible for defibrillator use, maintenance and training.
 

Action:
Identify affected devices (as detailed overleaf) and contact the manufacturer to arrange service and upgrade as soon as possible (as advised by the manufacturer to its distributors). The manufacturer is offering loan devices whilst the device is being upgraded.
