Device:
Baxter Colleague volumetric infusion pumps: all models.
 

Problem:
There are three problems that can cause the pump to stop and alarm resulting in an interruption to therapy:

  • battery undercharging
  • gearbox wear
  • false air-in-line alarms

There are two further issues with no accompanying alarms:

  • failure of upstream occlusion alarm
  • possibility of under-infusion by up to 19% (typically -5 to -9%, approximately 0.14% of UK pump channels may be greater than -9%).

All these problems present risks of death or serious injury.
