Light beam diaphragms for diagnostic X-ray equipment.
 
PROBLEM:
The MHRA has received several reports where light beam diaphragms (LBDs), also known as collimators, have become detached from X-ray tubes during routine clinical use, occasionally leading to patient injury.

All staff involved in the use and maintenance of radiology equipment.
 
ACTION:
Users and service agents should ensure that LBDs are regularly serviced, maintained and securely attached to the X-ray tube.
