Action 3 and Action 4 manual wheelchairs manufactured by Invacare

These wheelchairs have been on sale in the UK since 2004. All models are affected, including wheelchairs fitted with Comfort seating.

Invacare has revised the instructions for use for various safety related aspects.

Users who are unaware of these revised instructions are at risk of injury. In particular, from a fall due to instability or from positional asphyxiation due to poor lap belt position, fitting or adjustment.

The MHRA is aware of several reports relating to different aspects of the Action 3 and Action 4 wheelchairs. Injuries have been reported in relation to some of these issues. Invacare has revised the content of the instructions for use (IFU) for these models but has not provided them to all existing users. There is potential for serious injury to users or others if the revised IFU are not adhered to.

The Action 3 and Action 4 IFU have now been updated with the following additions:

  • Information on the correct fitting and adjustment of the tension-adjustable (contour) back rest upholstery.
  • Clarification on the set-up and configuration of the seat and rear wheel position.
  • Information on the need for and the use of a back brace for those wheelchairs with reclining back rests and/or tension-adjustable upholstery.
  • The correct fitting, use and method of adjustment of the various types of lap belts supplied with these wheelchairs.
  • A five year intended life cycle for these wheelchairs has been added.
  • Wheelchairs fitted with Comfort seating are not presently suitable for use as a seat in transport even though these wheelchairs were supplied with tie-down labels on the wheelchair frame (Invacare is working towards modifying these wheelchairs to allow use as a seat in a vehicle but have not yet been able to provide adequate detail or a date when it will be available).

The updated IFU for both models are available from Invacare UK or can be downloaded from the Invacare website (external link).

Invacare has already produced one Field Safety Notice (FSN 020) concerning the removal of tie-down labels on Action 3 wheelchairs. Invacare has also agreed to issue further Field Safety Notices for the Action 3 and Action 4 to cover the additional items shown above, but has not yet provided issue dates for these.

  • Trace all affected wheelchairs.
  • Ensure users receive a copy of the revised instructions for use.
  • Review the revised instructions for use to ensure that the wheelchairs are still appropriate for the existing users’ needs.
  • For affected wheelchairs fitted with a Comfort seating system, remove the tiedown labels from the frame.
