We have reviewed the types of incidents reported to us over the last two years and have revised this poster.
It is similar to the previous edition - quality and training have prime position at the top of the new poster as many of the incidents we see may not have occurred if formal procedures and appropriate training had been in place.
We make reference to the Department of Health’s document ‘Choice Framework for Local Policy and Procedures 01-06 – Decontamination of flexible endoscopes’ (external link). We would encourage you to familiarise yourself with these documents and take action where appropriate.
If you work in one of the devolved administrations you will need to consider the relevant documentation for your country.
We hope you will find this poster helpful and will bring it to the attention of all members of staff that need to be aware.
