Hospira has initiated a product recall due to the potential for under-delivery when these sets are used at flow rate settings of less than 10ml/hour. This MDA replaces MDA/2011/012 and MDA/2011/012R.

23 Feb 2011 | Medical Device Alert: Oxford Voyager fixed ceiling hoist system fitted with X-Y gate system manufactured by BHM Medical, supplied by Sunrise Medical or Joerns Healthcare (MDA/2011/020)
This Medical Device Alert has been issued because there is insufficient information supplied in the installation, inspection and maintenance instructions. This may cause the hoist to detach from the track during use resulting in serious injury to the occupant or others.
