Choose a payment method:
Cheque
Please send cheques, made payable to the Medicines and Healthcare products Regulatory Agency, to the Conference and Education Function, 16th floor Market Towers, 1 Nine Elms Lane, Vauxhall, London SW8 5NQ.
Bank Transfer
Bank of England, Government Counter, Threadneedle St, London E2R 8AH

Please ensure that you include the name of the conference and delegate name/s in the transmission slip and fax a copy to 0207 084 3522. If you experience any problems please use our alternative fax number 020 7084 2779.
Invoice
Please invoice my organisation marking the invoice for the attention of ______________ and quoting purchase order number ___________.
Within two working days of registering you will receive confirmation from the MHRA Conference and Education Team. If you have any difficulties placing your registration please call us on +44 020 7084 2903.
Confirmation
A confirmation will be emailed to you within 5 working days of receipt of this form. If confirmation is yet to be received after this time, please contact MHRA Conferences and Education. Fees must be paid in advance.
Substitutions and Cancellations
Substitutions are acceptable at any time. Written confirmation is needed for withdrawn bookings. If a suitable substitute cannot be found cancellation charges will apply.
Cancellation Policy
Over 21 days prior to the event a cancellation fee of £50 will be charged. 21 days or less prior to the event the full registration fee will be charged.
