To register for an event please complete the following online form or download the PDF version (below) and fax it back to the MHRA Conferences and Education team at +44 020 7084 3522.

Payment Method

(Please note that payment must be received prior to attending the event. Registrations will only be accepted if a valid PO or cheque is received in advance of the event.)

you must provide a valid Purchase Order Number to enable the MHRA accept this registration and raise an invoice.
NOTE: for FREE events you will still need to select a payment method, but the cost will be calculated on 0
     

     

Cancellation policy


Confirmation:  A confirmation will be emailed to you within 5 working days of receipt of this form. If confirmation has not been received after this time, please contact us via mhraconferences@mhra.gsi.gov.uk.

Substitutions and cancellations: Substitutions are acceptable at any time. Written confirmation is needed for withdrawn bookings. If a suitable substitute cannot be found cancellation charges will apply.

Cancellation charge for conference registration: Over 21 days prior to the event a cancellation fee of 50 + VAT will be charged. 21 days or less prior to the event the FULL registration fee will be charged.
